AVAILABILITY OF PASSPORT APPLICATION FORMS:

Passport Application forms are available for supply to applicants at Regional Passport Office, Chennai at nominal rates of Rs.10/- for Form No. 1,  Rs.5/- for Form No.2 .

Passport Application forms can also be obtained from selected
Speed Post Centres authroised for collection of passport applications
and also at the District Passport Cells set up in the office of the District Collectors.

FEE  STRUCTURE :

The following are the fees to be paid either by means of
cash at Counter or by demand draft drawn in favour of 
Regional Passport  Officer, Chennai payable at Chennai:

1.Passport (36 pages) of 10 years  validity           Rs. 1000/-
 

2.Passport to Minor (below 18 years) 
   validity for 5 years or up to 18 years
   whichever is less

3. Passport to Minors below 15 years                    Rs. 600/-
    with validity of 5 years or passport till

A FEW TIPS TO FILL-IN PASSPORT APPLICATION FORM:

1.    Normally you need four passport size photographs. One to be pasted on the original application and on the proforma for designated District Authorities attached to the application.  After filling up the application, a clear photocopy of the application may be made and the photograph should be affixed on the photocopied application and the proforma for the Designated Authorities.   Do not sign on these four photographs and also do not  staple these four  photographs.

2.    Write your full name in capital letters at Column 1
       of application as it should appear in the passport. 

        for example

Name: KARUR VAIKUNTA SUBRAMANIYAN RAMANATHAN
PILLAI

 
Write the Surname as "PILLAI " in the boxes provided
for surname and put a comma(,) and write

"VAIKUNTA SUBRAMANIYAN RAMANATHAN" as the
Given Name in the rest of the columns. In case
he do not
have a Surname just write the Given Name. No Initial
should be written and they should be expanded.

3.    Write full name of your parents with expansion of 
        initials and not like Mr. Chavan which is surname. Surname

should be mentioned after Given Name.

4.    If you are a male, write your wife's full name and 
        if you a female write your husband's full name
        against the column 'Spouse'. In case you are unmarried this

        column may be left blank.

5.    Write only visible marks of identification that are
        on your face, neck and fore-arms. If you do not have 
        any visible marks write 'NIL'.

6.    Give your correct place of  birth i.e.  Village/Town,
        District and State (all three details)

7.    Mention your profession clearly like Private Service
        /Self employed/Unemployed/Student/Employee in a Public
        Sector Undertaking/Govt. Servant (attach No Objection
        Certificate if you are a PSU employee or Government
        servant). Declaration duly acknowledged by his/her
        Head of office, to the effect that he/she has informed
       his/her intention to apply for a passport can also be
       attached. Persons serving in Defence Ministry / Establishment
       including Army, Navy, Air Force, Ordnance Factory etc. and  
       Government Servants working in sensitive departments like       
       Atomic Energy Commission, Indian Space Research Organisation etc.
       should furnish NOC and an intimation to their Head of Department
       will not be accepted.

8.    In case you had applied for passport at any time in the
        past, please mention the file number and/or date of
        application even if you have not received the passport.

9.    In case you are already holding valid or expired
        passport, please surrender the passport with a Xerox
        copy along with the application

10.    In case the passport bears a valid visa, please choose
        to submit in person at the counter and avoid sending
        the same by post.

11.   In case passport facilities were refused/denied/
        impounded or you were repatriated to India from abroad,
        please give details at the appropriate Columns.
        Do not suppress any  information as it will delay issue of
        passport and cause levy of penalty.

12.    Please put your signature with ball point pen and 
        not with ink pen (including micro roller tip ink pen)

13.    If you have resided at more than one place during the last
         one year , please furnish one additional photocopy of the
         application for each additional place of stay.  For example, if you
         have stayed at three places during the last one year , please
         furnish one original and three copies of filled in forms altogether.
         Sign and put thumb impression on each copy of the form.  Please
         remember that all copies require signature and thumb impression
         in ORIGINAL .

14.    Attach two copies of  documents attested by a Gazetted Officer in
          support of your stay and date of  birth.

15.   Children particulars are not to be included in Mother's passport.
         Please apply separately for children.

16.  If you are entitled for Emigration Check Not
        Required (ECNR), submit copies of relevant documents         

17. Attach a copy of valid passport of adult children, if any,
      in the case of  Senior Citizen who are aged 65 years
      or above (optional)

18. If you are a family member of serving Central or State
      Govt. servants, attach a declaration by the Govt. servant
      countersigned by his/her Head of Office confirming the
      fact that the applicant is a member of his her family
     (optional)

19. If you are a retired Govt. servant and have held while
      in service a Diplomatic/Official Passport, attach your
     cancelled old Diplomatic/Official Passport as a proof.
 

   
 When applying
in person       : All copies of documents have
  to be self attested. All original documents to  be produced
  at counter  which could be taken back after  verification
   by  counter officials

When applying through post :Copies of documents
attested by a Gazetted officer only to be sent to
Regional Passport Office, Chennai and NOT THE ORIGINAL DOCUMENTS. Please do not send applications by post to Passport Application Collection Centre/District Passport Cells.
  

When applying through a Representative: An application for a passport may be submitted through a representative carrying an Authority Letter as per specimen in the Passport Information Booklet attached to the application.  All copies of documents should be attested by a Gazetted Officer at the designated places and the Original should be shown for Verification.

                                                                                            

                                                                                       


SPEED POST PASSPORT SERVICES

Speed Post Passport Service has been introduced from 5th March
2001.  In Chennai City, the following Post Offices will accept
Passport Application Forms on payment of additional service
charge of Rs.100/- to the Postal Department apart from the
prescribed passport fee payable to RPO Chennai

Passport Application forms along with Speed Post Envelope
can also be had from the above Post Offices on payment
of Rs.20/-.

Applications for issue of fresh passport and for minors
below 18 years of age will alone be accepted in the above
post offices.

Applications submitted in the above post offices will be
processed in RPO Chennai with the same file reference number
given by the Postal Department.

Enquiries on applications submitted in Post Offices can
be had either at the Enquiry Counter of RPO Chennai or in
the website of RPO Chennai with same file reference number,
key number provided by the postal department.

 

                                                                                                          


ACCEPTANCE OF PASSPORT APPLICATION FORMS THROUGH
DISTRICT PASSPORT CELLS:

Acceptance of Passport Application forms through District Passport
Cells set up in the office of the District Collector has been introduced
w.e.f. January 2002.

1. Applications are also made available in District
Passport Cells through the retail outlets of Kendriya
Bhandar/Chennai Printers Industrial Co-operative
Society Limited.

2. Applications for issue of Fresh Passport and for
minors below 18 years of age and Reissue of Passport
in cases of expired passport will alone be accepted in
District Passport Cells.

3. Applications submitted in District Passport Cells will be
processed in Regional Passport Office, Chennai with
the same file number given by the District Passport Cells.

4. Enquiries on applications submitted in District Passport
Cells can be had either at the Enquiry Counter of RPO
Chennai or in the website of RPO Chennai with same file
reference number provided by the District Passport Cells.

5. Applicants need not pay any additional fee to District
Passport Cell other than the prescribed Passport Fee
by means of Demand Draft drawn in favour of Regional
Passport Officer, Chennai.

ARE YOU APPLYING FOR  THE FIRST TIME ? If YES, then

Please

1)  submit application form complete in all respects with
      four photographs (please do not staple photographs)

2) give proof of residence i.e.    two copies of
     a) Ration Card  or
     b) Statement of Running Bank Account or 
     c) Telephone bill or
     d) Electricity Bill or
     e) Water Tax Bill or
     f)  Voters Identity Card or
     g) Income Tax assessment order of last
          three years etc.

     h) Appointment Letter of Reputed Companies on letter head or
          spouse's passport copy, parent's passport copy (in case of    
           Minor)

3) give proof of date of birth i.e.
     a) Two copies of Birth Certificate or School
          Certificate
     b) If  illiterate, Sworn Affidavit from a Notary
          Public
     c) If literate and unable to produce any certificate,
          a sworn affidavit from a Notary Public / First class 
          Judicial Magistrate

4) No Objection Certificate (NOC), if Govt. servant/
     Public Sector Employee.  (Alternatively, a declaration
duly
     acknowledged by his/her Head of Office, to the effect that
     he/she has informed his/her employer of his/her intention to apply
    for a passport can also be attached)

Persons serving in Defence Ministry / Establishment including Army, Navy, Air Force, Ordnance Factory etc. and  Government Servants working in sensitive departments like Atomic Energy Commission, Indian Space Research  Organisation etc. should furnish NOC and an intimation to their Head of Department will not be accepted.

5)If you need 'ECNR' produce two copies of Degree 
     Certificate /Three year Diploma Certificate (If
     Provisional Degree/Diploma then submit two copies
     of all semester mark sheets also )  (Persons eligible to
     get ECNR Endorsement are listed separately)

6. If you are a Senior Citizen of 65 years or above, and  your adult children has any valid passport, please attach a photocopy of the said passport.

7. If you are member of serving Central or State Government  servants, attach a declaration by the Government servant   countersigned by his/her Head of Office confirming the   fact that the applicant is a member of his/her family.

8. If you are a retired Govt. servant and have held while in service a Diplomatic/Official Passport, attach your cancelled old diplomatic/official passport as a proof.

When applying in person: all copies of documents have to be
self-attested; all original documents to be produced at counter which could be taken back after verification by counter officials

When applying through Post: Copies of documents attested by a Gazetted officer to be sent to Regional Passport Office, Chennai.
Please do not send  applications by post to Passport Application Collection Centre/District Passport Cells.

REISSUE  OF PASSPORT 

Reissue of passport can be done at any time after expiry
of its validity and also even before one year
of its expiry.
 

HOW TO APPLY FOR RE-ISSUE ?

1. Submit application form (Form 1) duly filled in with two
    unsigned photographs (do not staple photos)

2. If the passport is expired more than one year ago, then photocopy
    the application form without affixing photo, signature and thumb
    impression and then affix photo , signature and thumb impression.

3. Surrender Old passport with two copies of the same
4. Submit Degree/Diploma Certificate, etc.  if 'ECNR'
    required (For Details see list of persons entitled
    for ECNR endorsement)
5.To include spouse name after marriage, provide two copies
    of  Marriage Certificate / Joint Sworn Affidavit with joint attested
    photo
6. Pay fee as per size of passport booklet - Rs.1000/- for
    normal booklet.

7. If there is any change in address, documents to prove your place of stay.  

 
						       


RENEWAL OF PASSPORT 

Passport issued with short validity (one year or five years)
can be renewed at any time after three months of its issue.

Short validity passport issued by other passport issuing 
authorities/offices can also be renewed at any time after
three months of its issue provided applicants reside in 
the jurisdiction of RPO Chennai and produce proof in
support of their stay.

HOW TO APPLY FOR RENEWAL ?

If the passport with short validity was issued by RPO
Chennai, apply in miscellaneous form (Form 2) with a
copy of  the passport without fee. If the passport
with short validity was issued by other offices, 
      * apply in miscellaneous form (Form 2)
      * submit a copy of the passport
      * No fee to be paid

 

                                                                                                

 


 

ISSUE OF DUPLICATE PASSPORT IN LIEU OF
LOST/DAMAGED PASSPORTS :

HOW TO APPLY?

1. Submit Duplicate Passport Application form (Form 1)
    duly filled-in.

2. Surrender Damaged Passport with two copies of relevant pages
3. If the passport was LOST, furnish two copies of 'FIR' lodged with police.  If 'FIR'    not available,  furnish Non-traceability Certificate issued by the Officer Inchage of the Police Station.

4. Furnish two copies of lost passport, if available
5. Fee applicable - Rs.2500/-

                                                                                         

 


HOW TO APPLY FOR PASSPORT AFTER CHANGE OF NAME  ?

COMMON TO  a - d  BELOW :

Please submit                

 i)    Application form (Form 2) duly filled-in with one
        photograph  
(please do not staple the photo)
 
ii) 
  Surrender present passport with two copies
 
iii)  Furnish two copies of proof of residence 
 
iv)  Fee -  Rs.1000/-

 a)     IN CASE OF NORMAL CHANGE IN NAME

         i)   Furnish original Newspaper cuttings giving change of
               name published in two reputed newspapers - one LOCAL NEWSPAPER of the area in which he/she is residing and the second Newspaper of the area of PERMANENT ADDRESS.

         ii)   Furnish an affidavit as per format given in Annexure 'F'
               of the instructions attached to passport application

  b)   FOR CHANGE OF NAME AFTER MARRIAGE

          i)  Two copies of Marriage Certificate issued by Registrar of Marriages       

                                             or

          ii)  A Notarised joint affidavit from the husband and wife.
               If unable to  produce joint affidavit, furnish an affidavit
               with wedding photograph wedding invitation card and
               a letter giving reasons for not being  able to give joint
               affidavit.  Marriage certificate issued by Church and
               Quasi's Certificate (with a certified translation) in case 
               of Muslims can also be given as proof of marriage.

c)   WHEN THERE IS A CHANGE IN NAME ON ACCOUNT OF   
       DIVORCE

Please furnish

         i)  Divorce deed duly authenticated by Court (or)
        ii)  Deed Poll  (or) Sworn Affidavit as per specimen in Annexure'F' given in the Instruction Booklet attached to the application.
         

d)   CHANGE IN NAME IN CASE OF REMARRIED PERSONS
       
(AFTER DIVORCE OR DEATH OF SPOUSE)


Please furnish

          i)  Divorce Deed or an affidavit in support of  applicant's
               Divorce or Death Certificate of the previous
               husband/wife, as the case may be AND  

          ii) Two copies of marriage certificate issued by the
               Registrar or  marriages in respect of recent marriage (OR)

         An affidavit from the wife and the  husband alongwith a Joint Photograph with present Spouse.

           iii) Photocopy of present Spouse's passport, if any.

 

                                                                                          

 


 

 

ISSUE OF PASSPORT TO MINORS :

Are you applying for passport for a minor? (Below 15 years)

Please furnish

1. A declaration from parents / Legal guardians affirming the particulars furnished in the application about the Minor Child not inclusion of the Minor in the either of Parent's passport etc. as per speciment in Annexure 'H'


2.Two copies  Birth Certificate/School Certificate for
    proof of date of  birth
    (Birth Certificate  is a must for those born on or after
      26.01.1989)

3.Two copies of Ration Card etc. or parents' passport copy for
    proof of stay

4. Attach passport copies if both parents hold passports 
    including ECNR/ECR page: Mother's passport should be in 
    married  name  or should have husband's name entered in
    her passport or father's  passport should have wife's name

5.If  both parents/one parent do not hold passport, furnish
   declaration   (format available in website)

6.If either of the parents is staying abroad, furnish a consent
    letter in the form of an affidavit attested by Indian Embassy
    concerned (format available in website - please see
   "Minor Affidavit if  parent(s)  abroad").

7. In case the parent is a divorcee, furnish an attested copy of
    Court   order giving  custody of the child.

8. Fee applicable - Rs.600/- only.

Are you applying for passport for children of age
above 15 years and below 18 years ?

* Apply in RPO, Chennai or in the Passport Office which covers 
     the Institute
* Write hostel address under present address
* Write parents' address under permanent address
*  Letter from Warden/Prinicipal for proof of stay at hostel, etc.
*  Usual proof of stay for parents' address
*  Four personal particular forms instead of two forms
*  Rs.600/- if  below 15 years;
    Rs.1000/- if above 15 years  and both parents do not
    hold valid passport(the passport  validity will be for 10 years):
    Rs.600/- if above 15 years and either of parent holds a valid       
    passport (the passport validity will be for 5 years)

Passports to Adopted Children

All matters relating to adoption is dealt by the Central Adoption 
Resource Agency (CARA) in the Ministry of Welfare, 
Government of India

Inter country adoption

The prospective adoptive parents abroad (foreigners), who wish to
adopt a child from India, have to approach the recognized foreign
agencies of their country for adoption. The child for adoption may
be taken from any of the child welfare agencies recognized by the
Government of India.

Documents required in connection with issue of passport in 
inter-country adoption cases:

1) Application form duly filled in
    (a) Certificate of Recognition of the local adoption agency by
         CARA
2. Court Order with a copy of photograph of  the child  attested
        by the Court
3. No Objection Certificate from CARA
4. Power of Attorney
5. Child Study Report
6. Financial Guarantee executed by the foster parents and 
    attested by Indian Mission abroad or Financial Guarantee
    executed by the local person with proof of solvency
7. Copy of  child surrender (relinquishing) document
8. Sworn Affidavit  for Date of  Birth executed by the Orphanage
9. Legal Guardianship affidavit (as per format)

Adoption of a child living with biological/natural parents by any 
Indian National living abroad/foreign national of Indian Origin 
living abroad.

Documents required for issue of Passports:

1. Adoptive parents' passports copy
2. Legally valid adoption deed registered with Registrar of
    Documents
3. Proof of  stay of adoptive parents
4. Birth Certificate of  child

In case of adoption by Muslim, Christians and Parsis, the
adoption can be done under the provisions of Guardians 
and  Wards Act,  1890

Documents required for issue of passport for the adopted child

1. Court Order (with photograph of the child) for
    guardianship  and for taking the child out of the country
2. Passport copy of adoptive parents
3. Guardianship affidavit
4. Proof of stay of adoptive parents
5. Birth Certificate of the child 

Adoption of a child by Indian Hindus from a child welfare
organisation

Documents required for issue of passport:

1. Court order (with photograph of the child)  permitting 
    the child to be adopted by the parents
2. Legally valid registered adoption deed
3. Parents'  passport copy
4. Birth Certificate of the child or copy of surrender document
     from  the orphanage

In case  place of adoption falls outside the jurisdiction of 
RPO Chennai:

If a child is taken for adoption from a place which does not falls under the jurisdiction of Regional Passport Office, Chennai, the adoptive parents should submit  passport application of that child with  proof of  residence and two
sets of  personal particulars forms giving the address of place of  residence where from adoption took place and the present residence. Passport shall  be issued after obtaining police report from both the places where the adoption took place and the place of residence of  adoptive parents.

Do you need Police Clearance Certificate (PCC) ?

Present  Passport issued by RPO, Chennai:

  • Apply in Miscellaneous form (Form 2) mentioning the country for which PCC is required with fee of Rs.300/- and the original passport

  • Ascertain from RPO, Chennai whether police report received on Your present passport;  if not, submit two personal particulars forms

  • The I.T.Professionals who require Police Clearance      Certificate(PCC) are requested to submit two personal particular(PP) forms with their application clearly indicating the police station under whose jurisdiction they reside.  PP forms are required in cases where the passport office does not have police report in respect of the passports issued by RPO Chennai and in case of passports issued by other passport offices

Present Passport issued by other office:

* Apply in Miscellaneous form (Form 2) 
   mentioning  the country for which PCC is required with
   fee of  Rs.300/-
* Submit the passport along with a copy of  it
* Submit two personal particulars forms duly filled in
* submit a copy of proof of residence within the jurisdiction of 
   RPO Chennai

* Apply in Miscellaneous application form (Form 2) with

Emigration Check Not Required Stamping on Passports

Following persons are entitled to get the ENCR stamp on their
passports. 

Gazetted Government servants, Income Tax payees who paid
income tax for the last One year or possessing PAN CARD. 

Salaried  people may submit income-tax deduction statement
in Form16 for three years from their employers.  Businessmen
and others should submit Income-Tax clearance certificates for
three years issued by the Income Tax Department in the
prescribed format with tax paid challans

Persons holding professional degrees such as 
           Doctors holding MBBS Degrees or
           Degrees in Ayurved or Homeopathy, Accredited Journalists,
           Engineers,Chartered Accountants, Cost Accountants,
           Lecturers, Teachers, Scientists, Advocates.

Persons staying abroad for more than three years
Spouses of the above mentioned categories and their dependent
children below 24 years

Persons  with 10 + 2 qualification from a recognized Central / State Board or possessing Higher Degrees from recognized Central / State Universities.

Seamen in possession of Continuous Discharge Certificate and
Sea Cadets and Deck Cadets

       (i) who have passed final examination of three year B.Sc
            Nautical Sciences  courses at T.S. Chanakya, Mumbai or
      (ii) who have undergone three months Pre-Sea training at
            any of the Government approved Training Institute
           after production of Identity cards issued by the Shipping
           Master, Mumbai/Calcutta/Chennai Persons holding
           permanent Immigration Visas or any country
           eg. Green Card in USA

Persons possessing  two years' Diploma from any Institute recognized by the National Council for Vocational Training (NCVT) or State Council of Vocational Training (SCVT) or persons holding three years' Diploma / equivalent Degree from Institutions like Polytechnics recognized by the Central / State Governments.


Nurses possessing qualifications recognised under the
Nursing Council Act,1947

All  persons above the age of 50 years

Children below 18 years or age provided they are accompanied by any or both parents.


Even if you do not have the ECNR Stamp,  you can visit
Bangladesh, Pakistan and all countries in Europe (except the
Commonwealth of  Independent States),  North America,
Australia, Japan, New Zealand  without emigration clearance.
 
In any case,  Protector of Emigrants can grant emigration 
clearance to certain categories of skilled/semi-skilled workers
for a period of  six months if application is made through
recruiting agent and  there is a valid employment visa. In case,
the person concerned  is leaving the country for non-employment
purposes, he can obtain suspension from emigration clearance
for the specified country and for the specified period from
the Protector of   Emigrants

           Office, Chennai, please quote the following:

           a) File number/Key number and Date of application

You can either include your husband's name in your  passport
 as an endorsement  or change your name by including your 
 husband's name in your name replacing your father's name

 Can I apply for services in Passport Office, Chennai?

Irrespective of the issue of passport by any Passport Office,
 
you can apply to Regional Passport Office, Chennai  provided
 you are presently residing in the jurisdiction of RPO., Chennai
 for any services relating to passport.

What is the difference in procedure between applying 
  in person and by Post?

While you apply in person two copies of all the documents 
 duly  self attested have to be handed over at counter 
 along with the  original
which will be returned after
 verification at counter  immediately.

While applying by post, you have to submit two copies of 
  all the
 documents duly attested by a Gazetted Officer
  bearing his name seal
and address of his office.

I am a student staying away from my parents. Where do I
 apply  for passport and what proof of residence shall I
 have to furnish?

(a)    For instance, if you are a student staying in Coimbatore 
           (which falls under the jurisdiction of R.P.O.,  Chennai), 
           and your parents are staying  in Pondicherry/Chennai,
           you can apply in Passport Application Collection center,
           in Pondicherry/R.P.O., Chennai and you have to
           provide four personal particulars forms.  Please
           furnish your hostel  address/the place where you are
           staying in Coimbatore  as present address and your
           parents' address as permanent address

(b)    In case your college falls under the jurisdiction of some
           other  Passport Office,  you can either apply to the
           Passport Office in  whose jurisdiction your 
           hostel/place of stay is located or in the Passport Office
         in whose jurisdiction your parents' address
           (permanent address)  is located

(c)     For proof of residence at your hostel/place of stay
           either you should   provide a letter from the  Principal/
          
Warden or proof of residence of the guardian with
           whom you are staying along with the guardianship
           affidavit.  You should also provide proof
           of address for your parents' address (i.e. permanent
           address)

(d)    Please submit two attested copies of all the documents


 
What is the difference between Renewal and Re-issue? 

Renewal  :

Renewal pertains to extension of the validity of a passport
 which is valid generally for one year or five years.

Extension of validity is given on the same passport booklet.

 
The applicant is, therefore, advised to fill in a miscellaneous  
 application form without fee and ensure that the passport
 is not  punched which will render it invalid.

Reissue  :

Reissue means issue of a fresh passport booklet after expiry
 of ten years validity.

The applicant is advised to fill in Form 1 form which is used for
 applying passport for the first time.
